Understanding the Context

An oral fibroma is a common benign scar-like reaction to persistent long-standing irritation in the mouth. It is also known as a traumatic fibroma, focal intraoral fibrous hyperplasia, fibrous nodule or oral polyp. Nuchal type fibroma (see comment) Comment: Hypocellular haphazard bundles of dense collagen are seen entrapping native adipose and nerves. Proliferative nerve sprouts (similar to those found in a traumatic neuroma) are present.

Supported by the location of the mass, these features are diagnostic of nuchal type fibroma. The pathological entity described is an irritation fibroma. Irritation fibromas are the most common oral reactive lesion. While irritation fibromas would be positive upon immunohistochemical staining with vimentin, they are negative with S100, SMA, CD34 and other soft tissue markers.
